Transforming Healthcare: Stellantis and Conifer Health’s Collaborative Program Enhances Employee Well-being and Access to Care
Stellantis was facing the challenge of high disease burden risk among its plan participants, with low primary care utilization and high emergency room visits. Through proactive outreach with Conifer, Stellantis successfully implemented strategies such as near-site clinics, telemedicine visits, and coordination with other benefit partners to help members manage their conditions and navigate care. Conifer Health also facilitated monthly meetings, health fairs, and provided health newsletters to improve member health literacy.
